Declaration of Intent Form Needed For Application to Supervised Practice Programs
Students who are completing DPD requirements and applying to accredited dietetic internship programs or Individualized Supervised Practice Programs will need a Declaration of Intent form submitted by the program director at the time of application. The Declaration of Intent form states that at the point of the application submission, the student is projected to successfully complete the requirements for completion of the program and for attainment of the Verification Statement for Didactic Program in Nutrition and Dietetics.
Consequently, the requirements for submission of the Declaration of Intent as part of the internship application process are the same as the requirements for attainment of the Verification Statement.
